Have seen Criss Angel do that one on his show on A&E a few times and a few different versons. Never seen the "participants" do the trick however.

The only I can think of is that there were more people in on it than we were meant to believe. The restaruant may have had a special table placed there for him or that girl that held the salt shaker may have been in on it??
